The incident happened on Friday in Rukumba Cell Rutare Sector, Police confirmed.
Superintendent of Police Christopher Semuhungu said it happened when five people went in to extract the limestone (locally known as Ingwa) which is used by rural constructors to paint houses.
Semuhungu attributed the accident to heavy rains that have softened the ground, making such underground quarries susceptible to collapse.
He urged residents to avoid illegal underground extraction operations and join licensed groups that guarantee safer working conditions.
The deceased were identified as Valentin Muhire, Mukamukunzi Jeanne, Alex Sebahutu and Liliane Nzayituriki.
The survivor, Jean Paul Nyakamwe, is receiving treatment in hospital.
Semuhungu also urged local leaders to step up the sensitization of residents to avoid dangers, especially illegal quarries and mines.
